Organization Description:
Asera Care Hospice of Omaha provides hospice support services to terminally ill patients and their families in their residence - whether that is in their own home, a nursing home or assisted living facility. RN Case managers manage the medical care working directly with patients and their caregivers, working under the direction of the patient's doctor. IN addition, according tot he desires and needs of the family, we provide home health aides, social work support, spiritual care and bereavement services, and volunteer support.
