Evercare Hospice & Palliative Care provides end-of-life care to patients with a terminal illness and a prognosis of six months or less. An interdisciplinary team of doctors, nurses, certified nursing aides, chaplains, social workers, bereavement coordinator and volunteers provides care to the patients and their families at the patient's place of residence.
