Heartland Hospice Services

Organization Description:

Hospice programs are designated to offer support, care and comfort to terminally ill patients in the final states of life as well as their families. Provided either in home or within an assisted living or skilled nursing center, our Hospice Care program includes palliative care by hospice professionals, counseling for patient and family as well as pastoral support. In addition registered nurses, therapist, dietitians and social workers are available to meet all the patients’ health care needs.

Hospice is a promise … a promise to patients who are facing the most difficult time of life, when choices seem few, and the patient and family situation is most fragile. We offer comfort to the body, mind, and spirit.
