Heartland Hospice Services

Organization Description:

Hospice is a compassionate approach to caring for those who are faced with a life-limiting illness. Hospice provides patients and their families with supportive services that not only keep the patient comfortable and educate the family, but also provide emotional support.

Many people think that hospice is a place, but really it is a philosophy of care. Whether a patient is living in the home he or she has owned for years, a nursing home or assisted living, hospice care can be tailored to fit the patient‚†‚’‚‚¢‚€‚š‚¢‚€‚š‚‚¢‚€‚™s and family‚†‚’‚‚¢‚€‚š‚¢‚€‚š‚‚¢‚€‚™s unique situation.

The typical hospice team consists of the patient‚†‚’‚‚¢‚€‚š‚¢‚€‚š‚‚¢‚€‚™s physician and the hospice physician; registered nurses; social workers; spiritual care coordinators; bereavement counselors; dietitians; pharmacists; physical, occupational and speech therapists; home care aides; and volunteers. Some patients even benefit from music, art and massage therapists, as well.

Access to hospice is based upon a documented life-limiting prognosis by a certified physician and the patient‚†‚’‚‚¢‚€‚š‚¢‚€‚š‚‚¢‚€‚™s decision to elect to receive hospice benefits. Hospice is a 100 percent-covered benefit under Medicare. Hospice care is also covered under Medicaid and many insurance plans.

Mission Statement:

Heartland Hospice provides specialized care that meets the physical, emotional and spiritual needs of patients and loved ones facing the challenges of an advanced illness. The quality of life for those we serve is enhanced by encouraging personal choices, supporting meaningful experiences and preserving dignity.

We are committed to providing comfort of the mind, body and spirit. Our specially trained staff helps manage pain and symptoms, thereby enabling patients to live their lives as fully as possible.

Our team members create a comforting environment and provide emotional support to families who need help coping with the loss of a loved one.

To serve as our guiding principles every step of the way, we've created the Heartland Caring Promises.

We offer hospice services wherever a patient calls home, whether the care is provided in a home setting, assisted living facility or skilled nursing center. In some locations, we offer Heartland Hospice House -- an inpatient care center for patients who need assistance managing pain or stabilizing their condition before returning home.
