Organization Description: 

Heights and Hills supports and empowers over 3,000 frail and homebound older adults and their caregivers in nineteen Brooklyn neighborhoods. Through individual case management and transportation services, we help seniors remain independent and fully-functioning vital members of their communities for as long as possible, and provide critical support to their caregivers.
