Organization Description: 

The VNA Hospice of Philadelphia enables patients with a serious, progressive illness to live at home as fully and as comfortably as possible wherever home may be, even a nursing home or personal care facility. The hospice team includes nurses, social workers, home health aides, a chaplain, on-call nurses, volunteers and a bereavement coordinator. Palliative care goes beyond traditional services providing expert pain/medication management, pastoral care, respite care, and counseling to patients and their families in their homes.
